- Calls are scheduled for a consistent, set time of the day. Each call to the Coachee is 15-20 minutes with their individual life/mental health coach, followed by post-call documentation by the coach (approx. 30 min total).
- ALL calls are transcribed, recorded, AND confidential, except the elements of the calls addressing a concern. During the documentation period, changes will be notated and discussed with management when a notable change occurs, to determine whether to continue to monitor (minor sounds “off”) or to immediately contact the Team Leader (anything more than minor). The Team Leader will receive a call from the Phone Home coach or management.
- It is the responsibility of THE TEAM LEADER to follow up on the stated concerns with medical or
other appropriate personnel.
Team Leaders Can Be Coached Too
So, how I do I begin, ...you ask?
Call and schedule a free 30-minute consultation to determine if this will be a good fit for the
Coachee. Based on the interest survey pages, we will match the Coach and Coachee. The
Coachee then has a free 20-minute call to get to know each other and see if they “click;” if not,
we will try another coach. We have an intake phone meeting to determine step/frequency and
schedule the consistent appointment time. Calls begin within 2 business days of receipt of
payment.

Why Are All Calls Recorded and Transcribed?
We utilize this technology for several purposes:
o For transparency purposes, especially when working with the elderly
Recorded
o To have audio documentation of possible changes or concerns
Transcribed
o To aid the coaches in remembering earlier conversations’ details
o To provide printed documentation to bring to medical appointments
Who Has Access To The Recordings and Transcriptions?
Active access is available to the coachee’s coach(es) and Phone Home
management.
o The coachee may request copies of the written transcripts.
o The team leader, health care proxy, and/or medical personnel will only have
access to the documentation needed for medical purposes; full conversations
are kept confidential expect when medically necessary, OR someone is in
danger of harming themselves or others.
How Are Steps Determined?
This is multi-faceted including, but not limited to; o Background given by the Team Leader, including daily living abilities and diagnosis when applicable o Observations during the in-take process o May change based on interactions with their coaches
Are We Locked Into Those Packages?
If you find the quantity of calls per week needs to be adjusted, we will certainly recalculate the rate based on the need. Brief re-evaluations occur frequently, determined by the step level. If a change in condition requires a change of package, we can do so at any time.
What Happens When the Coachee Passes Away?
• Phone Home will cancel the service effective the date of passing.
• Team leaders and their families are given 3 sessions of grief coaching.
• Soon after, Team Leaders will receive a keepsake recording of compiled
conversations by their loved one.
• Copies of transcriptions and recordings will be maintained as long are legally
required; thereafter, the documentation will be securely removed from our
system.
